The Brockton and Area Family Health Team (BAFHT)

Is inviting applications for a

Nurse Practitioner

This is a full-time contract position, for a period of up to one year.

 

The successful applicant will work with a number of local physicians and a group of interdisciplinary health care professionals at our Walkerton site.

 

The BAFHT provides primary healthcare services to the residents of Chesley, Durham, Mildmay, Paisley, Walkerton and surrounding areas.  These services include chronic disease management and health promotion and disease prevention.  The BAFHT uses a team approach that brings together family physicians with other healthcare providers, and is designed to reduce wait-times and improve community access to healthcare services at the local level.  Forming partnerships with local health and community service providers, the BAFHT works collaboratively in meeting the healthcare needs of the local population.

 

Roles and Responsibilities:

  • Work collaboratively with physicians and inter-disciplinary team members to effectively manage patient needs.
  • Provide individual patient assessment and treatment – health assessment, ordering of laboratory and DI screening tests, diagnosis, treatment and follow-up.
  • Provide health promotion and disease prevention counselling.

 

Qualifications and Competencies:

  • Baccalaureate degree in Nursing.
  • Certificate of Registration in the Extended Class from the College of Nurses of Ontario, and eligible to practice as a RN-EC.
  • Experience in a Community Health or Primary Care setting.
  • Demonstrated ability to communicate effectively with patients, families, and members of an interdisciplinary medical team.
  • Experience in patient education and chronic disease management.
  • Proficiency in the use of computers and information technology.
  • Valid drivers license and access to a reliable vehicle.
