Congestive Heart Failure
A 3-week educational program focused on managing congestive heart failure. Topics will include risk factors, medications, stress management, exercising safely, and dietary considerations. Presented by Dietitian, Social Worker, Pharmacist, Kinesiologist, and RN.
Healthy Heart
This 4-week program provides information and support regarding risk factors of heart disease, signs and symptoms of heart disease, medications, stress management and exercising safely.
Senior Fitness Test (Enrolled Patients only)
This functional fitness test will help you to identify your current physical strength, flexibility, agility, balance and endurance all of which are needed to perform everyday activities in later life and to enjoy an active, independent retirement. Open to ages 60 – 94.
Prevention/Early Detection of Cancer & Heart Disease
A 2-hour session providing information on what you can do to reduce risk of some cancers and heart disease, and learn early detection strategies. Prevention and Detection is your first line of defense. A joint presentation by Public Health, Ontario Breast Screening Program, and the Brockton and Area Family Health Team.
Fall Prevention
This 4-part program addresses bone health, the effects some medications may have on stability, activities for strength and balance and identifying fall risks in the environment. Includes a falls risk screening.
Chronic Pain Self-Management Program
People often assume their pain is the sole cause of their symptoms. However, each symptom contributes to other symptoms creating a vicious cycle. Learn how to break this cycle and gain control back into your life! CPSMP is a 6-week program.
Just the Basics - Nutrition
Learn how some simple modifications to your food choices and eating patterns can improve your blood sugars.
Just the Basics - Activity Class (NEW)****
Activity is key to successful weight loss as well as in the prevention and management of diabetes, arthritis, high blood pressure or cholesterol. Let us help you get started.
Cholesterol Program
Learn what your numbers mean and what you can do to improve your cholesterol levels and reduce your risk for cardiovascular disease.
Blood Pressure/Hypertension Class
Blood Pressure check as well as an information session to increase understanding, prevention, and control of hypertension.
Arthritis Rehabilitation & Education Program
Minimize the impact of arthritis on your life! Topics include: dealing with pain, arthritis medications, exercise, reducing joint stress and much more. (Offered in partnership with the Arthritis Society)
Breast Feeding Clinic
Provided by a RN/Certified Lactation Consultant that teaches the fundamentals of lactation and provide support on how to feed your baby. Helping mothers and babies with latching difficulties, painful nursing, low milk production, or inadequate weight gain. Service provided at the Public Health Unit by appointment only, please call (519) 881-1920.
Strength & Balance Exercises (Enrolled patients only)
An introduction to the use of free weights, resistance bands and balance activities. This 6-session program is intended for older adults. Appropriate for individuals with osteopenia or osteoporosis.
Healthy You Weight Management
A 10-week group program to liberate you from your diet. Learn how nutrition, physical activity, self-esteem, the environment and your behaviors affect your weight.
Medication “Check Up” Days (Enrolled patients only)
An opportunity to review your medications, and ask questions of our Pharmacist. By appointment only.
Functional Fitness for Older Adults
Tips for maintaining your physical independence. What types of exercises/activities are appropriate? How much, how often, and how hard?
“Quit Nic” - Smokers Helping Smokers
A 6-week support group for those who are considering quitting, those who are currently working at quitting and those who have quit.
"Break Through" Osteoporosis
A four week program that covers the role of diet, exercise and medication in the prevention and management of osteoporosis.
