Chronic Disease Management Programs
Cardiovascular Disease – Assist individuals and their families who have had a heart attack, angina, cardiovascular surgery or are at risk for coronary artery disease.
Congestive Heart Failure – Group programs, individual counseling regarding exercise, menu planning, medication issues and general lifestyle issues. Self referral and physician referral.
Hypertension – Group sessions and individual counselling. Self referral and physician referral. 90 minute information session to increase understanding, prevention and control of hypertension and decrease risk of heart attack and stroke.
Diabetes – All three FHN’s involved with Partnerships for Health initiative. As well, Healthy Living with Diabetes groups, Intensive insulin management programs and collaboration with dietitian, social work and pharmacy for individuals available.
Mental Health – Physician referral required for individual, couple and family counselling.
Stanford Chronic Pain Self-Management – Partnership with SWCCAC and Grey Bruce Chronic Disease Program Management. This workshop runs 2 ½ hrs per week for six weeks. You will receive the book “Living a Healthy Life with Chronic Conditions” and “Chronic Pain” Self referral and physician referral. Will meet with pharmacist and nurse in individual pain assessment prior to group program.
Memory Clinic – Our goal is to help you identify and manage your memory issues early, helping you maintain your independence as long as possible through early treatment and proper supports.
COPD – Group education classes and individual assessment available. Physician and self referral.
Cholesterol Program – Find out what your numbers mean and what you can do to improve your cholesterol levels and reduce your risk of heart disease.
"Break Through" Osteoporosis Canada – This 4 week program covers the role of diet, exercise and medication in the prevention and management of osteoporosis.
Health Promotion and Disease Prevention Programs
Smoking Cessation – Program with individual counselling, phone counselling and support group available. Physician and self-referral.
Nutritional Counselling – Cholesterol group program, weight management, and individual counselling available. Physician referral required.
Lactation – Partnership with Public Health and Brockton Area Family Health Team to provide a lactation consultant (from BAFHT) to the Public Health weekly Breast Feeding Clinic.
Weight Management – Registered nurses, dietitians and kinesiology collaboration, involving both individual and group sessions.
Falls Assessment – Involves kinesiology and/ or Registered nurses. Once a concern is identified, patient may be referred to CCAC for physiotherapy, occupational therapy and nurse follow up. Physician and self referral.
Arthritis Collaboration – Arthritis Society and BAFHT partnering to bring an Arthritis clinic to the Paisley clinic to serve our population. May be physician or self referred.
Alzheimer’s Society – First Link – Referral for assistance with Dementia diagnosis. Physician and self referral.
Pharmacist Services – Individual sessions to review your medications and ask questions of the pharmacist. Physician or self referral.
Chiropody Services – A chiropodist or chiropody assistant will help you with basic foot care or assist with treatment of basic foot problems. Physician referral, rostered patients.
Just the Basics – Nutrition – 90 minute session focusing on healthy eating habits. Find out what small changes you can make that will help you manage your weight, prevent or manage diabetes, or simply improve your nutritional health. Class runs monthly, rotating across the sites, depending on registration.
Just the Basics- Activity – 90 minute session to help you incorporate activity into your lifestyle. Class is ideal for individuals with, or at risk for, diabetes, obesity, hypertension, high cholesterol, or heart disease. Find out how much and what kind of activity is needed. Includes tips for staying motivated so your health will benefit from a more active lifestyle. Class to run monthly, rotating across the sites, depending on registration.
Just the Basics – Emotional Health– 90 minute session offering basic strategies to decrease stress in your life, improve communication skills, avoid negative self talk, and establish and maintain emotional balance in your life. CLass to run monthly, rotating across the sites, depending on registration.
Functional Fitness – 60 minute session to assist older adults with tips that will help you maintain your physical independence.
Strength and Balance – An introduction to strength and balance activities.
Quit Nic – Support group – "smokers helping smokers".
Smoking Cessation – Individual support and information.
