Alzheimer’s Society – First Link

Referral for assistance with Dementia diagnosis for both patient and family. Physician and self referral.

Arthritis Collaboration

Arthritis Society and BAFHT partnering to bring an Arthritis clinic to the Paisley clinic to serve our population. May be physician or self referred.

Breastfeeding Support

The Brockton and Area Family Health Team offers support, information and management for breastfeeding questions, concerns or difficulties to new or expectant parents.

Chiropody Services

A chiropodist or chiropody assistant will help you with basic foot care or assist with treatment of basic foot problems. Physician referral, rostered patients.

Complete Hearing Clinic

Professional hearing healthcare services including:
– hearing tests
– hearing aid recommendations and fittings
– counselling for the hearing impaired and their caregiver

COPD Activation Program

A brief community-based COPD self-management program that includes specific COPD education, medically supervised exercise sessions, and development of an ongoing exercise program to continue at home. Lead by a professional team of health care providers including a Registered Respiratory Therapist, Registered Kinesiologist, Registered Nurse and Physician Assistant. Participants will gain the knowledge and skills necessary for properly managing COPD and improving their overall quality of life.

2-1 hour education sessions
6-1 hour exercise sessions

Self or HCP/Physician Referral

Craving Change

A how-to workshop for changing your relationship with food. Understand why you eat the way you do, learn to comfort yourself without food, change your thinking, and change your eating. 4 sessions, 2 hours each.


Provide education and support to help you manage this chronic disease. As well, Healthy Living with Diabetes groups, Intensive insulin management programs and collaboration with dietitian, social work, pharmacy, and physician for individuals available.

Diabetes Screening/BP Check

Join FHT staff for a spot Blood Glucose Test, and a Blood Pressure Test. Education will be provided on ways to reduce risk, and prevention and treatment strategies will be discussed.

Flex, Stretch and Balance 50+

Group exercise program offered for 6 weeks with a focus on flexibility, balance, strength and general conditioning.
