Chronic Disease

Are you at risk for, or have you been diagnosed with, a chronic disease? The BAFHT offers patient education, self-management support, and routine monitoring to help you with the prevention, early diagnosis and management of chronic diseases. If your Primary Care provider is seeing emerging signs of cardiovascular disease, diabetes, metabolic syndrome, or another chronic disease, we can help you focus on developing and maintaining lifestyle behaviours that keep you healthy and active longer.

ELIGIBILITY Individual appointments and community and group educational sessions available to all members of the public.

CHANGE Program

If you are at risk for, or have been diagnosed with, Metabolic Syndrome, the Canadian Health Advanced by Nutrition and Graded Exercise (CHANGE) Program can help. The CHANGE Program runs for 1 year to offer you support to make long-term, preventative changes. A personalized nutrition and fitness program will be created to target your risk factors for heart disease, stroke, and diabetes and reduce the need for medication. You will meet quarterly with a Nurse Practitioner, and weekly during the first 3 months with our Registered Dietitian/Kinesiologist.

Community Outreach

Would you like a BAFHT team member to visit your group/organization/workplace to provide an education session about Chronic Disease management? We can work with you to provide a session that is both informative and relevant to the interests of the group.

Foot Care
Are you in need of foot care? The BAFHT Chiropodist can assess and treat acute and chronic foot conditions that may arise as you age or as a result of diseases such as gout or diabetes, and provide education to help you self-manage your foot health.

Priority is given to patients with diabetes with an ulcer, history of ulcers, ingrown toenail OR patients with diabetes with thick, painful, hard to trim nails who lack means to access private footcare services OR patients with an acute foot care issue e.g. pain, ingrown toenail or corn.


Kinesiology
Interested in improving your well-being through physical activity? If you have a mobility disorder, a strength and balance issue, a musculoskeletal injury, a chronic disease, or osteoporosis, we can help. BAFHT Kinesiologists offer one-on-one consultations and physical activity programs to help you move better, live longer and be healthier. They can assess your current level of function, develop a plan to improve your well-being, and set you up for long­ term health benefits.

Prevention Through Exercise

Research continues to show how beneficial exercise is for our physical and mental health. This informative exercise program is appropriate for anyone looking to maintain or improve their strength, range of motion or balance. This program is planned specifically to be safe for those experiencing musculoskeletal pain or living with chronic conditions.
This program is offered Monday-Friday from 10-10:30am online via ZOOM.

Team UNBREAKABLE

Running and physical activity have well-known physical and mental benefits. Team UNBREAKABLE, a Cameron Helps program, is a physical-health-for-mental-health-program for teenagers, started in Toronto and implemented locally by the BAFHT team. Our Team UNBREAKABLE program is an annual event that provides an opportunity for teenagers to train with BAFHT staff, at a comfortable and achievable pace, in preparation for a 5-km run. A weekly presentation on mental and physical wellness is provided and social inclusion is encouraged.

Seniors' Care
Geriatric Medicine Outreach Clinic

To enhance the care of frail elderly individuals, a Geriatrician and Geriatric Nurse Educator provide comprehensive geriatric assessment, recommendations, and follow-up to the patient, care providers and families. This service is offered in conjunction with Grey Bruce Specialized Geriatric Services.

Cognitive Assessment Team

To enhance the care of seniors, a Physician, Geriatric Nurse Educator, and Social Worker provide cognitive assessment testing, recommendations, and follow-up to the patient with their care providers and families. This service is offered in conjunction with Grey-Bruce Specialized Geriatric Services and the Alzheimer’s Society of Grey-Bruce.

Mental Health

Are life stressors or emotional issues getting in your way of living life well? Our Social Workers and Counsellors offer counselling services for anxiety, depression, substance abuse, relationship stress, mood disorders and other life stressors. We offer counselling for individuals, couples and families, and work with you to establish goals, develop strategies, and work toward individual and relationship wellness.

Nutrition

Thinking about improving your nutrition? The BAFHT Dietitians provide clinical nutritional assessments and interventions for a variety of nutrition-related conditions including diabetes, heart disease, celiac disease, IBS, IBD, electrolyte disturbances, micronutrient deficiencies, infant nutrition, and bariatric surgery– through individual appointments and group workshops. Nutritional education is provided on a variety of topics, including healthy family eating, maternal nutrition, introduction of solids, picky eating and sports nutrition. Our dietitians also offer cooking classes and food demos at different times throughout the year so keep an eye on our facebook for details.

Mindful Eating Group (To Replace Craving Change)

Offered at various times throughout the year, the Mindful Eating program is an 8 week group program to help you heal your relationship with food. You will learn about the benefits of mindfulness, and how to practice mindful eating, as well as become more in tune with your body and learn to respond to your natural hunger cues. This program is open to any adult (rostered or not rostered) and the program is being offered virtually during the pandemic. Pediatric Nutrition Groups

Join our Dietitians to learn more about introducing solids to your infant, learn some strategies about helping your child become an adventurous eater, expand their diets, and get some meal and snack ideas for you and your children. You will have an opportunity to have your nutrition questions answered. This program is being offered virtually during the pandemic and is open to all caregivers; follow our website or Facebook page to find out when the next program is being offered.

Community Outreach

Pre/Post Natal Support

Pregnant? Recently given birth? Hope to breastfeed or are breastfeeding? The BAFHT offers support and information to help you adjust to parenthood and healthy family living – before and after your baby arrives! We offer prenatal classes in two convenient formats and, once baby is here, we’re available for postpartum support, information, help and answers for breastfeeding questions, concerns or difficulties. Eligibility for these services varies; please see below.

Prenatal Care

The BAFHT Prenatal Classes are facilitated by a Registered Nurse and a Kinesiologist, and offer information on keeping healthy during pregnancy, prenatal and postnatal exercises, labour and delivery, breastfeeding, infant care and behavior, safety, adjustment to parenthood and healthy family living. Choose from a 1-day condensed Saturday Workshop or a Four-Session Series Workshop offered one evening a week for 4 weeks.

ELIGIBILITY For all expectant mothers and their partners/support persons.

REQUIREMENTS No referral required, but please register in advance.

Postpartum Recovery Program

If you’ve recently given birth, our 6-week Postpartum Recovery Program can help you recover from childbirth, restore and retrain pelvic floor and core muscles, strengthen large muscle groups, and safely return to exercise. Learn how to optimize nutrition to assist with recovery, and understand perinatal mental health and resources available.

ELIGIBILITY Appropriate for anyone who has given birth, or anyone living with incontinence, prolapse, pelvic pain or pain during intercourse.

REQUIREMENTS No referral required, but please register in advance.

Early Years Lactation Workshop

Breastfeeding information and support sessions provided by a Certified Lactation Consultant at the Ontario Early Years Centre in Walkerton.

Breastfeeding Education and Support

Are you an expectant or new nursing mother? If you have any questions before baby arrives, are unable to get your baby latched on well, your nipples become sore, cracked or bleeding, the engorgement does not go away after 48 hours or if you are not sure that breastfeeding is going well, a Certified Lactation Consultant can provide breastfeeding education and support.

ELIGIBILITY Individual appointments are available to all expectant and new mothers.

REQUIREMENTS No referral required, but please register in advance.

Nutrition

If you are a new or expectant parent and have questions about your own nutrition during pregnancy or postpartum, or if you have questions about your baby’s nutrition, we are here for you. You can reach out to one of our Registered Dietitian’s directly if you have any questions or would like an appointment, or to join one of our prenatal or pediatric nutrition group talks.

Primary Care

Working in collaboration with the BAFHT physicians, the Nurse Practitioners provide primary care visits focused on wellness, prevention, chronic disease management, diagnosis and treatment.

Telemedicine Program

Working in collaboration with the BAFHT physicians and the Telemedicine Coordinator, the Telemedicine Nurse Practitioner provides primary care visits that can include: comprehensive health assessment; monitoring and management of chronic diseases such as diabetes, arthritis, asthma and heart disease; prescribing and monitoring of medication; immunizations; annual physical exams; episodic illness care; referral to specialists and other human services; health teaching; smoking cessation; injury prevention; falls prevention assessment and information for older persons; and consultation and service coordination.

ELIGIBILITY Individuals who require a primary care provider and/or connection with CMHA-GB. Program services are available to individuals aged 18 years or older and who have either a serious mental illness or addiction issue (excluding tobacco as a primary addiction).

REQUIREMENTS Referral from CMHA counsellors/workers, ER physicians and primary care providers.

Respiratory Health

Having difficulty breathing? If you are living with respiratory disease, our goal is to improve and prolong your quality of life, and to decrease hospital and emergency-room visits through ongoing support. Our Respiratory Therapist can monitor, evaluate, treat and counsel patients with respiratory and cardio-respiratory disorders, such as chronic obstructive pulmonary disease (COPD), asthma, lung cancer, interstitial lung disease, cystic fibrosis, sarcoidosis or other respiratory diseases. We offer referrals for Spirometry testing, provide assistance with inhaler technique, and promote a healthy, active lifestyle through patient education and exercise classes.

COPD Activation Program

For individuals with mild to moderate COPD, our Activation Program may improve exercise tolerance, decrease disease progression, enhance your understanding of COPD and improve your ability to self-manage your disease. This self-management workshop series includes specific COPD education, medically-supervised exercise sessions, and the development of an ongoing exercise program to continue at home. Led by a team that includes a Registered Respiratory Therapist, Registered Kinesiologist and Registered Nurse. This program includes 6 combined sessions of education and monitored exercise.

Smoking Cessation

Are you interested in quitting smoking or reducing your tobacco intake? Quitting smoking or reducing your tobacco intake is the single most important thing you can do to improve your health and quality of life. We can help! We provide individual counselling and support to help you meet your smoking cessation goals. Through the Smoking Treatment for Ontario Patients (STOP) Program sponsored by the Centre for Addiction and Mental Health and the Ministry of Health Promotion and Sport, a BAFHT Nurse Educator can provide you with free nicotine replacement therapy and support to help you stop smoking cigarettes or reduce your tobacco intake.

ELIGIBILITY Available to all members of the public.

REQUIREMENTS Referral from Family Physician or Nurse Practitioner preferred but not required.
